Transition

Transition means change—and at Independence Associates, we’re here to help you navigate it confidently. Our Transition Services support people with disabilities who are moving from one stage of life or care into another, whether it’s from an institutional setting into the community or from school into adulthood.

Two Key Areas of Transition

We focus on two main types of transition, both centered on empowering individuals to live independently:

1. Transition from Institutional Settings to the Community

We support individuals who are moving out of nursing homes, rehabilitation facilities, or other institutional settings and into community-based living. Our team works closely with each person to identify the resources and support needed to make this shift safely and successfully. This includes help finding accessible housing, arranging for in-home services, and developing daily living skills.

2. Transition from School to Adulthood

We also assist youth with disabilities as they move from school life into adult roles and responsibilities. This might include preparing for higher education, entering the workforce, learning to manage finances, or gaining the skills to live independently. We meet each young person where they are and help them develop a plan that fits their goals and needs.

Individualized Support for Every Journey

Every transition is unique. That’s why our Independent Living team works one-on-one with each consumer to provide personalized support and guidance. Together, we focus on:

  • Identifying goals and planning next steps
  • Building daily living and self-advocacy skills
  • Connecting to community resources and supports
  • Ensuring a smooth, safe, and sustainable transition
